Student Registration

Online for all students!

 

 

 

To access online registration: Skyward Family Access

Computers will be available at Streator High School to complete online registration if needed.  Staff will be available to assist you.  Every Tuesday, Wednesday and Thursday from 9:00 a.m. - 1:00 p.m. starting on June 7.

Registration Fees

  • The registration fee for each student is $150 (Qualifying reduced fee families pay $75).  If your student is eligible for free or reduced fees, his/her school fees will be waived or reduced.  Please call SHS for details at 815-672-0545.

  • Free/reduced fee status:  If you were “Free/Reduced” status during the 15-16 school year, YOU MUST re-apply again this year.  The form can be downloaded during the online registration process.  This form must be physically turned in to SHS.


Guidance Counselor Appointments in July

  • There will be a member of the Guidance Department available on the following dates:

    • July 13, 14, 20 & 21

    • Aug 3 & 4

    • 9am - 1pm

  • As stated above, appointments can be made online.


Chromebook Insurance

  • Optional SHS Chromebook insurance is $35.00 per student.

  • Accidental breakage deductible - $0.00 - 1st incident; $30.00 - 2nd incident; $60.00 - 3rd incident

  • Lost device or damaged beyond repair - $100.00 - 1st incident; $150.00 - 2nd incident; $250.00 - 3rd incident.

Driver’s Education Fee / P.E. Fees

  • Driver’s Education fee is $200.  (Qualifying reduced fee families pay $100) This must be paid at registration and is mandatory for students taking Driver's Education.

  • P.E. Uniforms (Shirt/Shorts) will be for sale for $15.00 per set.


Yearbooks

  • Yearbook orders can be made online with the rest of the fees.

  • The cost is $45.


Student Parking Permits

  • Student Parking Permits are optional.  The cost is $15.

  • Orders can be made online with the rest of the fees.

  • The permits will be distributed at Student Orientation on August 16th.


Freshman / New Student Physical

  • All freshman and new students from out of state are required to have a physical completed on the "Certificate of Child Health Examination" form. This form will also work for a freshman sports physical. The "Illinois Sports Physical" form will not work for a freshman physical.

  • The Immunization Section must be completed and signed for by a health care provider.

  • The parent / guardian must complete and sign the "Health History" section.

  • The "Physical Examination Requirements" must be completed by a MD, DO, APN, or PA.

  • THIS MUST BE TURNED IN TO THE MAIN OFFICE BEFORE JULY 15th.


Seniors/Immunizations

  • All seniors will need to submit proof of having received the Meningitis Vaccine prior to August 16th.
